Your USB drive is sabotaging you. The HEN auto installer is incredibly sensitive to USB timing. If you have a USB drive plugged into the left-most port (closest to the Blu-ray drive), the PS3 may attempt to read a stale update.pup file from it instead of the internal HEN module. Solution: Remove all USB drives before booting. Only plug them in after HEN is successfully active.

This is a unique "feature" of the PS3. The firmware update process physically checks for a connection to the Blu-ray drive motherboard (the BD controller). If you have a console with a dead or disconnected Blu-ray drive, the auto installer will fail instantly on official firmwares.
